HbA1c High, But My Sugar Readings Are Normal
Usually the two numbers are not disagreeing — they are describing different parts of your day.

It is a genuinely confusing result. You have been checking your sugar, the meter keeps showing numbers you are happy with, and then the lab report says your HbA1c is 8.2%. The instinct is to assume one of them is wrong. Usually neither is.
Your meter shows you one moment. HbA1c is an average of every moment across roughly three months — including all the hours you were not testing. When the two disagree, the most common explanation is not a faulty test. It is that the hours you did not measure looked different from the hours you did.
First, put the two numbers in the same units
Before guessing at causes, it helps to see how big the gap actually is. HbA1c can be translated into an average glucose in mg/dL — the same units your meter uses. The conversion comes from the international ADAG study and is published by the NGSP (opens in a new tab), the body that standardises HbA1c testing worldwide.

Now take your own readings from the last three months and work out their average. If your logbook averages 130 and your HbA1c of 8.2% implies something near 190, that is a real gap of about 60 mg/dL — and it tells you there are hours in your day running much higher than anything you have recorded. That is a far more useful starting point than "my HbA1c is high."
The usual culprit: you are testing at the calmest time of day
Most people test fasting, first thing in the morning, because that is what the doctor asked for and it is the easiest habit to keep. Fasting sugar is also, for many people with type 2 diabetes, the best-behaved number of the day. The spikes happen after meals — and if you never test two hours after roti and salan, those hours simply never appear in your record.
How much do they matter? A study in Diabetes Care of 290 people with type 2 diabetes (opens in a new tab) measured glucose across the whole day and worked out where the excess sugar was coming from. In those with an HbA1c below 7.3%, post-meal rises accounted for around 70% of the total. Fasting glucose only became the dominant contributor once HbA1c climbed past roughly 10%.
In other words: the better your fasting number looks, the more likely it is that your HbA1c is being driven by meals you never measured. This is exactly the profile of someone whose readings look normal and whose lab report does not.
For two weeks, keep your usual fasting check but add one reading two hours after your largest meal of the day — dinner for most households. If those post-meal numbers are running well above your fasting ones, you have found your gap without needing any new equipment.
The hours nobody tests
There is a second unmeasured stretch: the night. Someone who tests at 7 a.m. and again before dinner has no information about the eight hours in between sleeping, or the late-evening period after chai and biscuits. Sugar that runs high from 10 p.m. to 3 a.m. contributes to HbA1c exactly as much as sugar that runs high at noon — it is just invisible to a morning-only routine.
This is one place a continuous glucose monitor genuinely earns its cost, if one is affordable: a single 14-day sensor will show you the overnight pattern that months of fingersticks would not.
Could the meter itself be reading low?
It can contribute, though rarely enough to explain a large gap on its own. Home meters are not held to laboratory precision. The FDA's standard for over-the-counter glucose meters (opens in a new tab) asks that 95% of results fall within 15% of a laboratory measurement, matching the international ISO 15197 requirement. At a true glucose of 180, a compliant meter may legitimately show anywhere from about 153 to 207.
That tolerance is per reading, and errors usually scatter in both directions. But a meter that runs consistently low — an old or heat-damaged batch of strips, an uncalibrated device, strips stored in a hot car or on a sunny windowsill — will quietly shift your whole record downward. If your gap is large and unexplained, take your meter with you to your next lab visit and check it against the venous sample drawn the same morning.
When the HbA1c is the number at fault
Sometimes it really is the lab number. HbA1c measures glucose stuck to haemoglobin, so anything that changes how long your red blood cells live changes the result without your sugar changing at all. Iron deficiency anaemia and B12 or folate deficiency both push HbA1c up — a relevant possibility in Pakistan, where anaemia is common — and some inherited haemoglobin variants confuse certain testing machines. This is worth investigating, but it is worth investigating second. Check whether your testing schedule explains the gap before concluding that your blood chemistry does — the schedule explanation is far more common, and it is the one you can act on immediately.
What to bring to your doctor
The American Diabetes Association's Standards of Care (opens in a new tab) states that consistent and substantial discordance between glucose values and A1C results should prompt follow-up to find the underlying reason. That follow-up goes much faster if you arrive with evidence rather than an impression.
- Your actual readings for the period the HbA1c covers — with the time of day each one was taken, not just the value.
- At least a couple of weeks that include post-meal checks, so your doctor can see whether meals are the gap.
- Which meter and strips you use, and roughly how old the strips are.
- Anything that might affect your red blood cells: known anaemia, thalassaemia trait in the family, a recent transfusion, pregnancy, or kidney disease.
- Any recent illness, steroid course, or medication change — steroids in particular raise blood sugar sharply and often go unmentioned.
A high HbA1c with normal-looking readings is not a contradiction to be argued away. It is a prompt to look at the part of your day you have not been looking at — and most of the time, that is where the answer is sitting.
A record that keeps the time alongside each reading is what turns "my sugar seems fine" into a pattern you and your doctor can actually read against your lab report.
Frequently asked questions
My HbA1c is high but only slightly above my estimated average — is that still a problem?
A small gap is expected and not worth chasing. The conversion between HbA1c and average glucose is a population average, so individual results scatter around it, and a home logbook is a much smaller sample than the continuous monitoring the conversion was built from. It is a persistent, sizeable gap — one that shows up on repeat tests months apart — that warrants investigation, not a single result a few points off.
Should I stop trusting my glucometer?
No. The meter is answering a narrower question than the lab, and answering it well enough to act on. What a discordant HbA1c usually means is that you need readings at different times, not a different device. Compare your meter against a lab sample once if you want reassurance about the device itself.
How soon after changing something will my HbA1c improve?
Partially within about a month, more fully by two to three months, because the test reflects red blood cells of varying ages and the more recent weeks weigh more heavily. If you start controlling post-meal spikes in September, an HbA1c in early October will already have moved somewhat, but a December test is the fairer measure of the change.
